If you do not wish to have the data you share on this form stored, you can request for it to be deleted at any time by contacting MAC Clinical Research.

Applications to remote roles
By applying to roles that are home based you are confirming that you have a set up at home that allows you to work comfortably. This includes an appropriate desk and chair (excl. IT equipment which we will provide).
